We assure you that no data is being collected for third parties, or for our own use, unless you specifically choose to request information from us, and supply us with the necessary information to reply to your request. If you visit our office as a patient, you will be given a printed copy of our practice privacy policy regarding your personal Protected Health Information (PHI.)

Registration

If you e-mail us requesting information, we do not retain or use your e-mail address in any form of a database. After we reply to your e-mail, your address is deleted from our file and, unless you come to our office as a patient and supply us with the address again, we will not maintain a record of it.

Internet Provider (IP) Addresses

IP addresses are numbers assigned to your computer when you sign on to the internet, and are necessary for computers to communicate with each other. We do not maintain a list of, or utilize IP addresses, nor share them with any third party.
